Usage of Vardhamana Pippali Rasayana and its Benefits

Usage of Vardhamana Pippali Rasayana and its Benefits

This Rasayana is explained in the 3rd pada or Karaprachitiya Rasayana padam of the first chapter of Chikitsa Stana of Charaka Samhita. In this the Pippali i.e Piper longum or the long pepper is used in the vardhamana karma. Though Charaka in his Sootra Stana has specified not to use Pippali in excess, he has prescribed a large dose of pippali to be consumed in this context. Seeing the form of  prescription and the mode of administration the concept doesn’t look contradictory.

Mode of use:

On the first day 10 pippali’s should be consumed. From there onwards the number of pippali’s should be increased by 10 every day(i.e 20 pippali’s on 2nd day, 30 on 3rd day, 40 on 4th day…and so on). This consumption of pippali in ascending order shall be followed for 10 days. On the 10th day 100 pippali’s shall be consumed. From 11th day onwards the tapering of number of pippali’s starts and the number of pippali’s shall be decreased in the same proportion as they were increased. This consumption of pippali in descending order shall be followed for the next 10 days. So within a span of 20 days a total of 1000 pippali would have been consumed.

Form of consumption:
  • The people having uttama bala(good physical and mental strength, tolerance, courage) should take pippali in the form of pishta or kalka(paste)
  • The people having madhyama bala(moderate physical and mental strength, tolerance, courage) should take pippali in the form of shruta or kashaya(decoction)
  • The people having hrisva bala(less physical and mental strength, tolerance, courage) should take pippali in the form of choorna(powder)

Variation of dose or number of pippali’s in relation to Bala:

  • The people having uttama bala should start with 10 pippali’s, increase 10 pippali’s everyday until 10th day and decrease the same number of pippali’s each day for the next 10 days and finish with the starting dose of 10 pippalis on the last day. Total number of pippali’s consumed within 20 days in this format is 1000.
  • The people having madhyama bala should start with 06 pippali’s, increase 06 pippali’s everyday until 10th day and decrease the same number of pippali’s each day for the next 10 days and finish with the starting dose of 06 pippali’s on the last day. Total number of pippali’s consumed within 20 days in this format is 600.
  • The people having hrisva bala should start with 03 pippali’s, increase 03 pippali’s everyday until 10th day and decrease the same number of pippali’s each day for the next 10 days and finish with the starting dose of 03 pippali’s on the last day. Total number of pippali’s consumed within 20 days in this format is 300.

Diet restrictions during Vardhamana karma:

When the consumed pippali’s are digested properly Shastika shali odana(rice prepared using shastika shali or the rice grown in 60 days) should be taken as diet with cow’s milk and ghee.

Benefits of Vardhamana Pippali Rasayana:
  1. Rasayana: Rejuvenates and strengthens the tissues, provides immunity
  2. Brimhana: Promotes bulk and induces enormous strength
  3. Swara bheda nashaka: Allievates hoarseness of voice and other vocal disorders and makes the voice strong, bold and resonant
  4. Ayu-Hitakari: Prolongs the life span and enables one to live a disease free life
  5. Pliha nashini: Cures hepatomegaly and spleenomegaly
  6. Udara vinashanam: Alleviates abdominal disorders, especially ascitis
  7. Vayaha Sthapanam: Delays ageing process, restores youth and energy
  8. Medhya: Tones the brain, improves intellect and memory

RECTAL POLYP AND ITS AYURVEDIC MANAGEMENT

RECTAL POLYP AND ITS AYURVEDIC MANAGEMENT
Rectal Polyp
Rectal polyps are tissue growths that arise from the wall of the rectum and protrude into it. They may be either benign or malignant (cancerous).
Description
The rectum is the last segment of the large intestine, ending in the anus, the opening to the exterior of the body. Rectal polyps are quite common. They occur in 7-50% of all people, and in two thirds of people over age 60.

But some people with colon polyps experience
•           Rectal bleeding
•           Change in stool colour
•           Change in bowel habits
•           Pain, nausea or vomiting
•           Weakness, light-headedness, fainting, pale skin, and rapid heart rate due to iron deficiency anemia
•           diarrhoea or constipation
•           abdominal pain

Benign (non-cancerous) anal tumors
Polyps Polyps are small, bumpy, or mushroom-like growths that develop in the mucosa or just under it. There are several kinds.

         Inflammatory polyps start because of inflammation from injury or infection.

         Lymphoid polyps are caused by an overgrowth of lymph tissue (which is part of the immune system). Small nodules of lymph tissue are normally present under the anal inner lining.

         Hypertrophied anal papillae are benign growths of connective tissue that are covered by squamous cells. They are simply enlarged normal papillae, which are small folds of mucosa found at the dentate line. Hypertrophied anal papillae are also called fibroepithelial polyps.

Diagnosing and Treating Colonic Polyps
Your doctor may use a combination of tests to discover if you have colonic polyps. These tests may include
•           colonoscopy—a camera on a flexible tube is threaded through the anus to view the rectum and colon
•           sigmoidoscopy—similar to colonoscopy but shows only the rectum and lower colon
•           barium enema—an X-ray of the colon after it is injected with barium, which helps make images more visible
•           computed tomography (CT) scan—a tube with a camera, X-rays, and computers work together to create images of your large intestine. This is sometimes called a virtual colonoscopy.
•           stool test

CAN POLYPS RECUR
Once a polyp is completely removed, its recurrence is very unusual. However, the same factors that caused the polyp to form are still present. New polyps will develop in at least 30 percent of people who have previously had polyps. Patients should have regular exams by a physician specially trained to treat diseases of the colon and rectum.
Bowel cancer risk
About 1 in 10 polyps will turn into cancer. These are called adenomas
AYURVEDIC MANAGEMENT OF RECTAL POLYP
AGNI KARMA
A number of cauterization methods have been shown to be effective for rectal polyps, but are usually only used when other methods fail. This procedure can be done using electrocautery, infrared radiation, laser surgery, or cryosurgery. Infrared cauterization may be an option for grade 1 or 2 disease. In those with grade 3 or 4 disease re-occurrence rates are high
 KSHARKARMA OR KSHARA SOOTHRA TREATMENT
Involves the applying of a sclerosing agent, such as apamarg kshar into the rectal polyps or tying kshara soothra. This causes the vein walls to collapse and the rectal polyps to shrivel up.
Benefits of kshar sutra and kshara karma therapy
•           Simple safe and sure treatment
•           No complications
•           No recurrence
•           Very economic
•           Very high success rate
•           Day care surgery

PILONIDAL SINUS AND ITS AYURVEDIC TREATMENT

PILONIDAL SINUS AND ITS AYURVEDIC TREATMENT
A pilonidal sinus (PNS) is a small cyst or abscess that occurs in the cleft at the top of the buttocks. It can cause severe pain and often becomes infected. If it becomes infected, it may exude pus and blood and emit a foul smell.
Pilonidal sinus causes
Most doctors think that ingrowing hairs cause pilonidal sinuses. Pilonidal means "nest of hair". It is common to find hair follicles inside the sinus.
Another theory is that pilonidal sinuses appear after an injury to the sacrococcygeal region (the region relating to both the sacrum [the lower vertebrae] and coccyx). During World War II, more than 80,000 soldiers developed pilonidal sinuses that required a hospital stay. People thought the sinuses were due to irritation from riding in bumpy vehicles. For a while, in the US the condition was actually called "Jeep disease".
Pilonidal sinus symptoms
The symptoms experienced by someone with a pilonidal sinus include the following:
•           Pain at the bottom of the spine.
•           Swelling at the bottom of the spine.
•           Redness at the bottom of the spine.
•           Draining pus.
•           Fever.
Who is affected?
Pilonidal sinuses are rare, affecting about 26 in 100,000 people each year.
They affect at least twice as many men as women. The average age for a pilonidal sinus is 21 in men and 19 in women.
They are less common in children and people over the age of 45.
Presentation
A PNS may be asymptomatic for some time prior to presentation. The majority of patients only present with the onset of symptoms, usually pain and discharge. Occasionally a painless lump or swelling may be discovered by the patient while washing, or the characteristic midline pits may be found during a routine physical examination. Symptomatic disease usually presents as an acute pilonidal abscess, a chronic pilonidal abscess or complex/recurrent pilonidal disease.
Acute pilonidal abscess
The patient notices increasing discomfort and swelling over a number of days and the pain may be severe by the time of presentation. On examination there is a localised fluctuant swelling in the midline of the natal cleft with overlying cellulitis. The area is exquisitely painful to touch and often simply the act of separating the buttocks to examine the area is intolerable for the patient.
Chronic pilonidal disease
It is common for patients to present with chronic pain and discharge, often with a history of up to two years[2]. On examination a single, or occasionally, multiple sinuses may be seen. Tufts of hair or other debris, such as clothing fibres, are often visible arising from the sinus. Localised oedema, swelling and inflammation may be present masking the underlying sinus.
Complex or recurrent pilonidal disease
This is due to reinfection in neighbouring hair follicles or chronic infection from entry of hair or debris into a postoperative wound.
How Can I Prevent Pilonidal Sinus Disease?
You can prevent recurrence of pilonidal sinus by washing the area on a daily basis with a mild soap, making sure all soap is removed, keeping the area completely dry, and avoiding sitting for long periods.

Kshar Sutra treatment in pilonidal sinus
Kshar Sutra is a medicated thread (seton) coated with herbal Alkaline drugs like Apamarga (Kshar) (Ash of Achyranthus ascera), Snuhi (Euphorbia nerufolia) latex and Haridra (Curcuma longa) powder in a specific order. This combination of medicines on the thread helps in debridement and lysis of tissues, exerts antifungal, anti bacterial, and anti inflammatory. Another mechanism proposed for the Kshara Sutra is that it destroys the residual glands in the epithelium.
Pathya-Apathya ( Diet and routine activities ) :

From the day of Kshara sutra threading procedure, light diet is advised. Patient is also advised to take plenty of fluids, blend diet, green vegetables, salad and seasonal fruits. Avoid non-vegetarian , spicy and fermented food . Do not strain during defecation. Sitting in warm water is useful for cleaning & pain relief. Take a walk at least 2 kilometers per day.

HEMORRHOIDS AND ITS AYURVEDIC TREATMENT

HEMORRHOIDS AND ITS AYURVEDIC TREATMENT

INTRODUCTION
The word arsas is derived from the root “Ru-gatau” after adding the suffinx  ‘Asuna’ which means to take life.It is an entity in which it trouble patient like a “mansa keela”, common people call them piles. Hemorrhoids often described as "varicose veins of the anus and rectum", hemorrhoids are enlarged, bulging blood vessels in and about the anus and lower rectum. Hemorrhoids are usually found in three main locations: left lateral, right anterior and right posterior portions. They lie beneath the epithelial lining of the anal canal and consist of direct arteriovenous communications, mainly between the terminal branches of the superior rectal and superior hemorrhoidal arteries, and, to a lesser extent, between branches originating from the inferior and middle hemorrhoidal arteries and the surrounding connective tissue

CAUSES

The exact causes of symptomatic hemorrhoids are unknown. A number of factors are believed to play important role including
1. Irregular bowel habits ( constipation or diarrhea)
2. Lack of exercise
3. Nutritional factor ( a low-fiber diet)
4. Increased intra-abdominal pressure (prolonged straining, an intra-abdominal mass, or pregnancy
5. Genetics, absence of valves within the hemorrhoidal veins,
 6. Aging.
 7. Other factors that are believed to increase the risk include
·         Obesity
·         Prolonged sitting
·         Chronic cough
·         Pelvic floor dysfunction.

CLASSIFICATION

A) On the basis of position-
1. Internal
2. External
3. Interno-external
 B) On the basis of symptoms-
 1. Grade I: No Prolapse. Just prominent blood vessels.
 2. Grade II: Prolapse upon bearing down but spontaneously reduces.
3. Grade III: Prolapse upon bearing down and require manual reduction.
4. Grade IV: Prolapse and cannot be manually reduced.

SIGN & SYMPTOMS:

The symptoms of pathological hemorrhoids depend on the type present. Internal hemorrhoids usually present with painless rectal bleeding while external hemorrhoids may produce few symptoms or if thrombosed significant pain and swelling in the area of the anus.
External
If not thrombosed external hemorrhoids may cause little problem. However when thrombosed they may be very painful with this pain typically resolves over 2 to 3 days. The swelling may take a few weeks to disappear, and after healing a skin tag may remain. If they are large and causing issues with hygiene, they may produce irritation of the surrounding skin and thus causes itching around the anus.
Internal
Internal hemorrhoids usually present with painless, bright red, rectal bleeding during or following a bowel movement. The blood typically covers the stool, a condition known as hematochezia, is on the toilet paper, or drips into the toilet bowl. The stool itself is usually of a normal colour. Other symptoms may include mucous discharge, a perianal mass if they prolapse through the anus, itching, and fecal incontinence. Internal hemorrhoids are usually only painful if they become thrombosed or necrotic.

COMPLICATIONS
·         Anemia
·         Thrombosed
·         Strangulated piles

MANAGEMENT
Conservative:
1. Prevention of constipation- Laxative- Triphala churna, Panchasakar churna, Haritaki churna, Abhayaarista.
2. Deepan pachan- Chitrakadi vati , Lavan baskar churna, Agnitundi vati.
3. Arshoghna-Sooranpak,Arshakuthar ras, Shigru guggulu.
4. Hot sitz bath- Nimba pathra kwath,Triphala kwath ,Panchawalkal kwath.
5.Rakta stambhak-Bol baddha rasa, Bol parpati,Kukutandatwak bhasma Praval pisthi.
6. Vran ropak-Jatyadi tail, Devadarvyadi velichenna,durvadi tailam
7. Vednahar-Madhuyastyadi tail, Triphala guggulu,Narayana Tailam

PROCEDURES:-

 1. KASHAR SUTRA LIGATION: It is a procedure in which medicated herbal coated thread are applied onto internal and external piles mass within 5-7 days withered piles mass falls off. Cure rate-97%
 2. CHEDAN KARMA: A number of surgical excision techniques may be used if conservative management and simple procedures fails. All are associated with some degree of complications including bleeding, infection, anal strictures and urinary retention, due to the close proximity to the rectum to the nerves that supply the bladder. There may also be a small risk of fecal incontinence, particularly of liquid, with rates reported between 0-28 percent.
 3. AGNI KARMA: A number of cauterization methods have been shown to be effective for hemorrhoids, but are usually only used when other methods fail. This procedure can be done using electrocautery, infrared radiation, laser surgery, or cryosurgery. Infrared cauterization may be an option for grade 1 or 2 disease. In those with grade 3 or 4 disease re-occurrence rates are high
 4. KSHARKARMA: Involves the applying of a sclerosing agent, such as apamarg kshar snuhi kshar, into the hemorrhoid. This causes the vein walls to collapse and the hemorrhoids to shrivel up. The success rate four years after treatment is ~70%
Benefits of kshar sutra therapy
·         Simple safe and sure treatment
·         No complications
·         No recurrence
·         Very economic
·         Very high success rate
·         Day care surgery
·         Useful in all age and cardiac patients

APATHYA
Chilies, Fried Foods, Maida product, Non-Veg, Paneer, Constipating foods, Constant sitting, Excessive Pressure in defeacation.etc.

PATHYA
Cow milk, Butter, Buttermilk, Wheat, Ghee, Rice, Green vegetable, Regular sleep, Exercise, Regular diet, Non suppression of natural urges etc

CONCLUSION

According to Ayurveda, Arsha is a Maharoga (major disorder) described as Dirghanubandhi (chronic) and Dushchikitsya (difficult to treat) in nature along with Tridoshik (vat-pitta-kapha) and Marmashri (vital weak spots) (Guda and Bhaga) conditions. Out of which Guda is a Sadyapranahar marmas. Ayurveda has immense potential to solve many challenging and unresolved problems of the medical world. Kshara sutra ligation takes less time and, if the patient has any associated systemic disorder, then the procedure can still be performed with proper prophylactic measures. Kshara sutra ligation and ksara karma treatment is much more beneficial in comparison with hemorrhoidectomy at maximum point.
