Ayurvedic perspective on Marmaghāta (marma injury)

Marmaghāta = trauma or injury to marma (vital points) described by Ācārya Suśruta (Su. Śārīra 6).

  • Marmas are anatomical locations where māṃsa (muscle), sirā (vessels), snāyu (ligaments), asthi (bone), and sandhi (joints) meet and are especially associated with prāṇa.

  • Injury to them causes pain, dysfunction, deformity, disability, or even death, depending on the type.

🔹 Types of Marmaghāta

Suśruta mentions 107 marmas, classified as:

  1. Sadyapraṇahara – fatal instantly (e.g., heart, brain).

  2. Kālantarapraṇahara – fatal later (weeks/months).

  3. Viśalyaghna – patient survives till foreign body is removed.

  4. Vaikalyakara – causes deformity/disability.

  5. Rujākara – causes severe pain.

🔹 Lakṣaṇas (Clinical features of Marmaghāta)

  • Vedana (severe pain)

  • Śotha (swelling)

  • Rakta-srāva (bleeding)

  • Aṅgavyāpatti (loss of function in organ/limb)

  • Mṛtyu (death, in fatal marmas)

🔹 Chikitsā Sūtra (Treatment principles)

1. Śodhana & Rakta-stambhana

  • Control of bleeding and purification of wound:

    • Raktastambhaka dravyas – Lakṣā, Lodhra, Śālmali, Yastimadhu.

    • Śītala lepa for cooling and hemostasis.

2. Śodhana of Vrana (wound cleansing)

  • Triphala kwātha prakṣālana

  • Pañcavalkala kwātha prakṣālana

  • Haridra, Nimba for cleansing & antiseptic effect.

3. Śamana & Vedanāhara

  • Śirovirechana / Nasya for head marma injuries.

  • Taila abhyanga & Upanāha in vāta-predominant pain.

  • Eranda taila, Bala taila, Mūrchita ghṛta for vatavyādhi-like symptoms.

4. Ropaṇa & Vraṇa cikitsā

  • Ghṛta preparations (Jatyādi ghṛta, Nimbādi ghṛta) for wound healing.

  • Madhū (honey) – antimicrobial & healing.

  • Taila siddha with Patola, Nimba, Haridra for external application.

5. Rasāyana for Marmas

  • To restore vitality after injury:

    • Aśvagandhā, Śatāvarī, Amṛta (Guḍūcī), Bala, Pippalī rasāyana.

    • Suvarṇa bhasma, Abhraka bhasma in small dose under supervision.

🔹 Specific Treatments Based on Doṣa & Site

  • Vāta-dominant marmaghāta → Sneha, Svedana, Mūrdhataila, Basti.

  • Pitta-dominant → Śītala dravya, Ghṛta, Dugdha preparations.

  • Kapha-dominant → Lekhana, Kṣāra-based medicines, Uṣṇa upakrama.

🔹 Modern Parallel

  • Marmaghāta correlates with traumatic injuries to vital structures (brain, spinal cord, heart, joints, nerves).

  • Ayurvedic treatment aims at:

    • First aid (bleeding control, pain relief).

    • Wound healing.

    • Preventing deformity/disability.

    • Restoring vitality with rasāyana.

✅ Summary:
Treatment of Marmaghāta in Ayurveda is comprehensive: first rakta-stambhana & śodhana (hemostasis & cleansing), then vedanāhara & vāta-pacification, followed by vraṇa-ropana and rasāyana for long-term recovery. Prognosis depends on the marma type (sadyapraṇahara vs vaikalyakara etc.)

Professional Ethics of an Ayurvedic Doctor: A Classical and Contemporary Perspective

Abstract

Professional ethics form the moral foundation of medical practice. Ayurveda, being both a science and an art of healing, has clearly outlined ethical standards for physicians in classical texts such as Caraka Saṃhitā, Suśruta Saṃhitā, and Aṣṭāṅga Hṛdaya. These principles emphasize compassion, purity, confidentiality, self-discipline, and dedication to patient welfare. In the modern context, these ancient ethical ideals align with bioethical principles such as beneficence, non-maleficence, autonomy, and justice. This article reviews the ethical codes prescribed for Ayurvedic doctors and compares them with contemporary medical ethics frameworks.

Introduction

Professional ethics safeguard the sanctity of the physician–patient relationship. Unlike mere technical skills, ethics guide the physician in exercising knowledge with responsibility, humility, and service-oriented attitude. In Ayurveda, the physician (Bhishak) is regarded as a healer, teacher, and moral guide. Thus, an Ayurvedic doctor is expected to embody integrity and compassion while also adhering to modern legal and ethical codes of medical practice.

Ayurvedic Foundations of Medical Ethics

1. Code of Conduct for Physicians

  • Caraka Saṃhitā (Sūtrasthāna 9/20–25):

    • The physician should be clean, truthful, disciplined, and free from arrogance.

    • They should treat patients irrespective of caste, wealth, or social position.

    • Greed, jealousy, and cruelty are prohibited.

  • Suśruta Saṃhitā (Sūtrasthāna 3/52–55):

    • A physician should avoid intoxication, gambling, hunting, and unethical pleasures.

    • Should remain devoted to study, patient care, and self-control.

  • Aṣṭāṅga Hṛdaya (Sūtrasthāna 1/23):

    • The doctor must possess purity (śauca), compassion (dayā), knowledge (jñāna), and practical skill (dakṣatā).

2. Duties Toward Patients

  • Confidentiality: Caraka states that a physician must not disclose a patient’s secrets. (Ca. Vimānasthāna 8/13).

  • Non-discrimination: Treatment should be given without considering wealth, caste, or enmity.

  • Compassion: The doctor should see the patient as a family member.

  • Service before reward: Payment or honorarium should not be the primary motive; welfare of the patient comes first.

3. Personal Conduct of the Physician

  • Daily regimen of self-discipline (Dinacharyā, Brahmacharya).

  • Abstaining from unethical behavior, harsh speech, or exploitation.

  • Maintaining both physical cleanliness and purity of mind.

Modern Bioethical Principles and Ayurveda

Modern Principle Ayurvedic Parallel
Beneficence (do good) Dayā (compassion) – treating with kindness and sincerity.
Non-maleficence (do no harm) Ahimsā – avoiding harm by wrong medicine or negligence.
Autonomy (respect patient choice) Informed advice in Ayurveda, explaining do’s and don’ts clearly.
Justice (fairness in treatment) Samatva – equal treatment irrespective of social or economic status.
Confidentiality Rogi rahasya rakṣaṇa – safeguarding patient secrets (Ca. Vimāna 8/13).

Professional Ethics in Contemporary Ayurveda Practice

  1. Clinical Governance: Maintaining standards of care as per NCISM and AYUSH guidelines.

  2. Evidence-Based Ethics: Integrating classical wisdom with modern research.

  3. Professional Boundaries: Avoiding misuse of physician–patient relationship.

  4. Legal Compliance: Registration under state boards, adherence to AYUSH laws.

  5. Public Responsibility: Engaging in health awareness, medical camps, and preventive care.

Challenges in Present-Day Practice

  • Commercialization of healthcare vs. service motive.

  • Balancing traditional ethics with medico-legal obligations.

  • Maintaining patient trust in an era of misinformation.

  • Documentation and transparency in Ayurvedic clinical practice.

Conclusion

Professional ethics are the lifeline of Ayurveda practice. The classical Acharyas not only prescribed medicinal therapies but also created a moral code for physicians that resonates with modern bioethics. An Ayurvedic doctor must balance knowledge with compassion, skill with humility, and tradition with contemporary standards. Upholding these values ensures the preservation of the dignity of Ayurveda and the trust of society.

Ayurveda Hospital Management: A Classical–Contemporary Approach

Abstract

Ayurveda hospitals represent a unique healthcare model where traditional healing wisdom meets modern institutional management. While Ācāryas emphasized the role of physician, attendants, medicines, and infrastructure in healing, contemporary hospital management focuses on human resource management, financial sustainability, quality assurance, patient safety, and accreditation. This article explores the Ayurveda-based principles of hospital management and their integration into modern healthcare administration, highlighting pathways for efficient, ethical, and patient-centered Ayurvedic hospital systems.

Introduction

Ayurveda is not only a science of medicine but also an art of institutional healthcare delivery. The foundation of Ayurvedic hospital management is embedded in Suśruta Saṃhitā, Caraka Saṃhitā, and Aṣṭāṅga Hṛdaya, where detailed guidelines are available for physicians, attendants, instruments, medicines, and patient facilities. In the present era, the management of Ayurvedic hospitals requires a balanced integration of these principles with modern hospital management practices, quality accreditation, and patient-centered care.

Ayurvedic Principles in Hospital Management

1. Chikitsā Chatuṣpāda (Four Essentials of Treatment)

  • Caraka Saṃhitā (Sūtrasthāna 9/3–5): Physician, medicine, attendant, and patient are the four essential factors for successful treatment.

  • Hospital management ensures these four pillars through structured HR (doctors, nurses, therapists), drug supply chains, skilled attendants, and patient engagement systems.

2. Suśruta’s Guidelines on Healthcare Facilities

  • Suśruta Saṃhitā (Sūtrasthāna 34/7–9): Advises the physician to set up hospitals in clean surroundings, with adequate light, water, instruments, medicines, and attendants.

  • These instructions parallel modern infrastructure planning, sanitation, safety, and infection control in hospital management.

3. Physician & Staff Ethics

  • Aṣṭāṅga Hṛdaya (Sūtrasthāna 1/22–23): A physician should possess knowledge, practical skills, purity, compassion, and communication abilities.

  • This aligns with modern HR standards, medical ethics, and patient counseling.

4. Swasthavṛtta & Preventive Care

  • Ayurvedic hospitals are not limited to treatment but also preserve health through:

    • Seasonal regimens (Ṛtucaryā)

    • Panchakarma therapies

    • Yoga and meditation programs

    • Health awareness camps

This supports the preventive and promotive healthcare wing of hospital administration.

Modern Perspectives in Ayurveda Hospital Management

1. Human Resource Management (HRM)

  • Recruitment and training of doctors, Panchakarma therapists, dieticians, yoga instructors, and support staff.

  • Development of competency-based modules as per NCISM (National Commission for Indian System of Medicine) guidelines.

2. Infrastructure & Facility Management

  • Dedicated Panchakarma units (Abhyanga, Svedana, Vamana, Virechana, Basti, Raktamokṣaṇa).

  • Separate OPD and IPD blocks, well-ventilated wards, Ayurvedic pharmacy, yoga halls, herbal gardens.

  • Biomedical waste management, fire safety, and accessibility.

3. Financial & Resource Management

  • Transparent accounting and budgeting.

  • Pharmacy management with GMP-certified drugs.

  • Public-private partnerships and AYUSH insurance schemes.

4. Quality Assurance & Accreditation

  • NABH Accreditation for AYUSH Hospitals.

  • Implementation of AYUSH Standard Treatment Guidelines (STGs).

  • Documentation of case reports and evidence-based practice.

5. Patient-Centric Management

  • Electronic Medical Records (EMR) and telemedicine integration.

  • Patient feedback systems.

  • Counseling sessions for diet and lifestyle.

  • Follow-up reminders and community health linkages.

6. Marketing & Outreach

  • Community engagement through health camps, Ayurveda Day celebrations, blogs, and social media.

  • Collaboration with wellness tourism, yoga centers, and integrative medicine facilities.

Comparative Table: Ayurveda vs Modern Hospital Management

Classical Ayurveda Principle Modern Equivalent in Hospital Management
Chikitsā Chatuṣpāda HRM, Pharmacy, Patient care protocols
Suśruta’s hospital setup Infrastructure planning & infection control
Physician ethics (Bhishak guṇa) Medical professionalism & clinical governance
Swasthasya Rakṣaṇam Preventive & wellness care units
Upasthātā (Attendant) Nursing and paramedical staff
Ārogya Rakṣaṇa programs Public health, awareness campaigns

Conclusion

Ayurveda hospital management is a holistic system that merges classical wisdom with modern healthcare standards. While Ācāryas highlighted the physician, medicine, attendants, and patient as the pillars of treatment, modern administration enhances these principles through structured HR, accreditation, financial planning, and patient-centered protocols. Integrative management of Ayurvedic hospitals can not only preserve traditional healing but also establish them as sustainable, evidence-based, and globally recognized healthcare institutions.

Ayurvedic Clinic Management: An Integrative Perspective

Ayurvedic Clinic Management: An Integrative Perspective

Abstract

Ayurvedic clinic management is a blend of classical principles described by Ācāryas and modern healthcare administration practices. Ayurveda emphasizes the holistic role of the physician, attendants, medicines, and facilities in ensuring the success of treatment, while modern management adds organizational structure, patient care protocols, financial planning, and quality assurance. This article reviews the Ayurvedic foundation of clinic management and integrates it with contemporary health management strategies for efficient functioning of an Ayurvedic healthcare unit.

Introduction

Ayurveda, the ancient Indian system of medicine, is not only a science of treatment but also a comprehensive healthcare system encompassing clinical practice, preventive strategies, and ethical guidelines. For a clinic or healthcare unit to function effectively, Ayurveda outlines essential factors (Chikitsā Chatuṣpāda) while modern health management emphasizes patient safety, satisfaction, and sustainable operation. The synthesis of these perspectives provides a blueprint for Ayurvedic clinic management in the 21st century.

Ayurvedic Foundation of Clinic Management

1. Chikitsā Chatuṣpāda (Four Essentials of Treatment)

According to Caraka Saṃhitā (Sūtrasthāna 9/3–5):

"Bhishak dravya upasthātā rogi iti caturvidhāḥ bhavanti chikitsāyāḥ pādāḥ"

Meaning: The physician, medicine, attendant, and patient are the four pillars of successful treatment.

  • Bhishak (Physician): Well-trained, knowledgeable, ethical, and compassionate.

  • Dravya (Medicines): Pure, available, affordable, and effective.

  • Upasthātā (Attendant/Staff): Skilled in patient care, disciplined, and empathetic.

  • Rogī (Patient): Faithful, cooperative, and disciplined in following instructions.

This principle directly correlates with modern concepts of clinical governance, HR management, supply chain of medicines, and patient compliance.

2. Arogya Rakṣaṇam (Preventive Care)

Suśruta emphasizes Swasthasya Swasthya Rakṣaṇam (preservation of health) as equally important as curing disease. An Ayurvedic clinic should thus:

  • Provide dietary and lifestyle counseling (Ahāra-Vihāra).

  • Conduct seasonal therapies (Ṛtucaryā, Panchakarma).

  • Promote community awareness programs (Ayurveda Day, Yoga Day).

3. Hospital & Clinic Setup in Classics

  • Suśruta Saṃhitā (Sūtrasthāna 34/7-9): Advises that a physician’s workplace should be clean, well-ventilated, equipped with instruments, medicines, attendants, and emergency facilities.

  • Aṣṭāṅga Hṛdaya (Sūtrasthāna 1/21): Mentions that the physician should reside near a king or community, indicating accessibility as a management priority.

Thus, classical texts suggest infrastructure planning, patient-centric design, and resource preparedness.

Modern Management Perspectives in an Ayurvedic Clinic

1. Human Resource Management (HRM)

  • Recruitment of trained Ayurvedic doctors, nurses, therapists (Panchakarma technicians).

  • Training programs for clinical protocols, communication, and patient empathy.

  • Integration of NCISM competency-based modules for clinical staff.

2. Financial & Resource Management

  • Budgeting for medicines, therapies, wages, and infrastructure.

  • Efficient pharmacy management (Ayurvedic dispensary).

  • Insurance integration and NABH accreditation for Ayurveda hospitals.

3. Patient Relationship Management

  • Transparent consultation process.

  • Clear prescription and therapy explanation.

  • Follow-up reminders, patient feedback collection.

  • Use of digital platforms (EMR, telemedicine, online booking).

4. Quality & Safety Protocols

  • Maintaining GMP-quality medicines.

  • Following Ayush Standard Treatment Guidelines (STG).

  • Infection control during Panchakarma.

  • Fire safety, emergency medicines, referral system.

5. Marketing & Community Outreach

  • Patient awareness through social media, blogs, Ayurveda camps, health talks.

  • Collaboration with wellness centers and yoga institutions.

  • Evidence-based documentation of case studies to build trust.

Integrative Model of Ayurvedic Clinic Management

Ayurvedic Concept Modern Equivalent
Chikitsā Chatuṣpāda HRM, Pharmacy, Patient care system
Swasthasya Swasthya Rakṣaṇam Preventive healthcare, wellness programs
Rogi (patient qualities) Patient engagement & compliance
Upasthātā (attendant) Nursing & support staff training
Bhishak (physician ethics) Clinical governance, professionalism
Infrastructure guidelines (Suśruta) Hospital management & accreditation

Conclusion

Ayurvedic clinic management is deeply rooted in classical wisdom, where the physician, medicine, patient, and attendant form the cornerstones of care. Modern management enhances these principles with structured HR, finance, quality assurance, and patient engagement. A well-managed Ayurvedic clinic not only treats disease but also ensures holistic well-being, sustainability, and community trust.

Dhama et al. (2021) പ്രസിദ്ധീകരിച്ച “Gut microbiome of Asian elephants: implications for health and conservation” എന്ന പഠനം ചെറുപ്രായത്തിലുള്ള ആനക്കുഞ്ഞുകളുടെ മലയിൽ Lactobacillus പോലുള്ള പ്രോബയോട്ടിക് ജീവാണുക്കൾ കൂടുതലായി കാണപ്പെടുന്നതായി വ്യക്തമാക്കുന്നു. ഇതോടൊപ്പം, Rahman et al. (2023) നടത്തിയ “Elephant dung as a potential source of antimicrobial and cytotoxic compounds” എന്ന ഗവേഷണത്തിൽ ആനയുടെ മലയിലെ ചില

ഗവേഷണപരമായ കണ്ടെത്തലുകൾ പ്രകാരം, animal musk derivatives ന് anti inflammatory, cardiovascular stimulant, neuroprotective action ഉണ്ടാകാമെന്ന് 2017-ൽ Journal of Ethnopharmacology പ്രസിദ്ധീകരിച്ച ഒരു ലേഖനത്തിൽ പറയുന്നു. 2020-ലെ Integrative Medicine Research പ്രസിദ്ധീകരണത്തിൽ traditional Asian medicine ൽ ഉപയോഗിച്ചിരുന്ന civet based formulations ചില adaptogenic, CNS supportive ഗുണങ്ങൾ നിർവഹിച്ചിരുന്നുവെന്ന് വിലയിരുത്തി.

Cervical Dysplasia: Ayurvedic Management, Treatment Protocol, and Natural Healing

Cervical dysplasia is a precancerous condition in which abnormal changes occur in the cells lining the cervix, often associated with Human Papillomavirus (HPV) infection. Modern medicine classifies it into mild, moderate, or severe dysplasia (CIN I, II, III) and considers it as a precursor to cervical cancer if untreated. Symptoms may include abnormal vaginal discharge, post-coital bleeding, and pelvic discomfort, though many cases remain asymptomatic.

From an Ayurvedic perspective, cervical dysplasia can be correlated with Yoni Vyāpada (uterine disorders), Granthi (cellular abnormalities), and Arbuda (precancerous growths), with an emphasis on Pitta–Kapha duṣṭi and āma accumulation. Ayurveda aims at correcting metabolism, clearing āma, balancing doṣas, and rejuvenating reproductive tissues to prevent malignant transformation.

Ayurvedic Perspective

  • Doṣa Involvement:

    • Kapha – abnormal cell proliferation, growth, and discharge.

    • Pitta – inflammation, ulceration, and precancerous cellular changes.

    • Vāta – pain, irregular bleeding.

  • Samprāpti (Pathogenesis):
Agnimandya (impaired digestion) → Āma formation → Pitta–Kapha aggravation → Srotorodha (channel obstruction in yoni) → Granthi/Arbuda-like changes in cervix → Dysplasia.

Ayurvedic Treatment Protocol for Cervical Dysplasia

1. Nidāna Parivarjana (Avoidance of Causes)

  • Avoid junk food, excess spicy/sour foods, alcohol, smoking, and irregular lifestyle.

  • Maintain genital hygiene, prevent repeated infections.

2. Śamana Cikitsā (Palliative Management)

  • Internal Medicines:

    • Kāñcanāra Guggulu – useful in precancerous growths, Granthi, Arbuda.

    • Arogyavardhinī vaṭi – detoxifies liver, improves metabolism, reduces āma.

    • Kaishora Guggulu – balances Pitta–Kapha, prevents infection.

    • Punarnavādi Maṇḍūra – reduces inflammation and corrects rakta doṣa.

    • Triphala guggulu – useful in granthi and abnormal cervical discharges.

  • Kaṣāya (Decoctions):

    • Varuṇādi kaṣāya – effective in pelvic granthi and abnormal growths.

  • Regular prāṇāyāma and meditation reduce stress, improve immunity.

Integrative Approach

  • Screening with Pap smear and HPV testing is essential to monitor progression.

  • Ayurveda helps in early dysplasia and supportive care in moderate cases.

  • Severe dysplasia (CIN III) and carcinoma-in-situ may need surgical intervention; Ayurveda can support healing, immunity, and recurrence prevention.

✅ Conclusion

Cervical dysplasia is a precancerous condition that can progress to cervical cancer if ignored. Ayurveda correlates it with Yoni Granthi and Arbuda, caused mainly by Kapha–Pitta duṣṭi with Āma. Treatment focuses on doṣa balance, āma pachana, detoxification, local therapies, and rasāyana support. Remedies like Kāñcanāra Guggulu, Varuṇādi kaṣāya, Triphala wash, Uttarabasti, and Rasāyana drugs play a vital role. With an integrated approach combining Ayurveda and modern screening, cervical dysplasia can be effectively managed and malignant transformation prevented.

Bartholin’s Cyst: Ayurvedic Management, Treatment Protocol, and Natural Remedies

A Bartholin’s cyst is a fluid-filled swelling that develops near the vaginal opening due to blockage of the Bartholin’s gland duct. Sometimes, it may get infected and form an abscess, causing pain, discomfort, and difficulty in sitting or walking. In modern medicine, treatment includes antibiotics, drainage, or surgery. Ayurveda provides a holistic approach for managing Bartholin’s cyst by reducing inflammation, balancing doṣas, removing obstruction, and rejuvenating reproductive health.

Ayurvedic Perspective

In Ayurveda, Bartholin’s cyst can be correlated with Yoni Granthi (localized swelling/growth in the female genital tract) and sometimes Upadansha–yoni arbuda depending on the clinical features.

  • Kapha doṣa – causes obstruction and cystic swelling.

  • Vāta doṣa – leads to pain and hardness.

  • Pitta doṣa – when infection develops, it produces burning, redness, and pus.

  • Samprāpti (Pathogenesis): Agnimandya (poor metabolism) → Kapha–vāta duṣṭi → obstruction of srotas (channels) in yoni → Granthi formation (Bartholin’s cyst).

Ayurvedic Treatment Protocol

1. Nidāna Parivarjana (Avoidance of Causes)

  • Avoid excessive oily, heavy, and cold foods.

  • Maintain genital hygiene.

  • Prevent prolonged sitting and excessive heat exposure.

Medicines like Kāñcanāra Guggulu, Varuṇādi Kaṣāya, Punarnavādi Maṇḍūra, local applications with Triphala kvātha wash, and cleansing therapies like Virecana and Basti play a key role. With proper lifestyle, hygiene, and Ayurvedic support, Bartholin’s cyst can be effectively managed and recurrence prevented.

Ayurvedic Management and Treatment Protocol for Adnexal Tumors

Introduction

Adnexal tumors are growths arising from the ovaries, fallopian tubes, or surrounding tissues. They can be benign or malignant, presenting with symptoms like abdominal pain, pelvic mass, bloating, menstrual irregularities, or infertility. From an Ayurvedic perspective, these can be correlated with conditions such as Gulma (abdominal mass), Granthi (localized growths), or Arbuda (tumors), depending on the presentation.

(Overuse / repetitive strain) –

🔎 Subheadings

(a) അഭ്യംഗം (Oil Massage)

(Sudation therapy)

(Dietary Regimen)

  • Pathya (Do’s):
  • Apathya (Avoid):

(Lifestyle Regimen)

  • Seasonal immunity dip: Monsoon season = increased incidence of flu, dengue, malaria.
  • Njavara rice (medicated gruel): Rich in antioxidants, immunomodulatory.
  • Herbs (ginger, tulsi, black pepper): Proven antimicrobial and anti-inflammatory action.
  • Oil massage & steam: Improves circulation, reduces stress hormones, boosts immunity.

🧪 Modern correlation: Studies show that seasonal dietary & detox regimens reduce inflammatory markers and enhance immune resilience against viral and bacterial infections common in the rainy season.

✅ Conclusion

Key Takeaways:

  1. Monsoon season → digestive power low, vata aggravated → requires regimen.
  2. Karkataka regimen = diet + oil therapies + panchakarma + lifestyle.
  3. Ayurveda + modern immunity science → seasonal wellness strategy.

Ayurvedic Treatment Protocol for Anal Fissures (Parikartika)

Anal Fissures – Ayurvedic Management & Treatment Protocol

1. Modern Overview

Anal fissure = small tear in the lining of the anus, usually caused by passing hard stools, constipation, or chronic straining.

  • Symptoms: Sharp pain during defecation, bleeding (fresh blood), spasm of anal sphincter, constipation, burning sensation.

  • Modern management: Laxatives, stool softeners, local anesthetics, sphincter relaxants, surgery in chronic cases.

2. Ayurvedic Understanding

Anal fissures are correlated with Parikartika described in classical texts.

  • Doṣa involvement: Vāta (cutting pain), Pitta (burning, bleeding), Kapha (chronicity, induration).

  • Nidāna: Ati rukṣa āhāra, vāta-pittaprakopaka ahāra, chronic constipation, strain.

  • Samprapti: Vāta prakopa → Kathina mala (hard stool) → tearing of guda mārgā (anal canal) → rakta srāva + sphincter spasm + vedanā.

3. Chikitsā Sūtra (Principles of Treatment)

  • Vāta-Pitta Shamana (relieve pain & burning).

  • Mridu Virechana (soft evacuation, prevent recurrence).

  • Bṛṃhaṇa & Ropana (healing of fissure).

  • Local Snehana & Avagāha Sveda (reduce spasm & pain).

  • Rasāyana (prevent chronic recurrence).

4. Panchakarma Protocol

  • Avagāha Sveda: Sitz bath with Triphala Kaṣāya, Pañcavalkala Kaṣāya → relieves pain, improves circulation, promotes healing.

  • Snehana & Basti:

    • Mātrā Basti with Kṣīrabala Taila or Yashtimadhu Taila (softening stools, healing fissure).

    • Anuvāsana Basti in chronic cases with Jatyādi Taila or Balataila.

  • Mridu Virechana: With Avipattikara Chūrṇa or Trivṛt Lehya to regulate bowel.

5. Oral Medications

  • For Constipation & Smooth Evacuation

    • Triphala Chūrṇa at bedtime.

    • Gandharvahastādi Eranda Taila (mild purgative).

  • For Healing & Pain Relief

    • Guggulu Panchapala Chūrṇa.

    • Abhayāriṣṭa 10–15 ml bd.
