Clinical Management Protocol – Giant Papillary Conjunctivitis (GPC) in Ayurveda

1. Ayurvedic Understanding

  • Probable Correlation: Kaphaja–Pittaja Abhishyanda

  • Doṣa Involvement: Kapha (kleda, srava, kandu) + Pitta (daha, raga, shopha)

  • Dūṣya: Rasa, Rakta

  • Srotas: Rasavaha, Raktavaha, and Netra srotas

2. Treatment Objectives

  • Reduce Kapha–Pitta duṣṭi in Netra.

  • Relieve inflammation, itching, and discharge.

  • Purify Rakta and enhance ocular immunity.

  • Prevent recurrence through lifestyle regulation.

3. Internal Medications

  • Kaṣāya / Decoctions

    • Paṭolakaṭurohiṇyādi Kaṣāya – 15 ml with warm water, twice daily before food.

    • Manjishṭhādi Kaṣāya – for Raktaśodhana, 15 ml twice daily.

  • Ghṛta / Medicated Ghee

    • Triphalā Ghṛta – 5 ml with warm milk at bedtime, for Netra prasadana.

  • Rasāyana & Mineral Preparations

    • Saptāmṛta Lauha – 1–2 tablets twice daily with honey and ghee.

    • Chandraprabha Vati – 1–2 tablets twice daily (if mucous discharge is excessive).

4. Local Therapies

  • Āścyotana (Eye Drops)

    • Triphala kvātha, Daruharidrā svarasa, or Yaṣṭimadhu kvātha filtered, cooled, and instilled 2–3 times/day.

  • Netra Prakṣālana (Eye Wash)

    • With Triphala kvātha or Yaṣṭimadhu kvātha once or twice daily.

  • Anjana Karma (Collyrium)

    • Srotośodhana Anjana with Saindhava–Elādi preparation, once at night.

  • Tarpana (Retaining medicated ghee over eyes) (optional in chronic/recurrent cases)

    • With Triphala Ghṛta or Mahatriphala Ghṛta for strengthening Netra dhatus.

5. Pathya (Diet & Lifestyle)

  • Favourable Diet (Pathya)

    • Light, cooling, and easily digestible foods: barley, green gram, old rice, leafy vegetables.

    • Use of ghṛta and milk in moderation.

    • Fresh Amla juice or Triphala powder at bedtime for Netra rasāyana.

  • To Avoid (Apathya)

    • Excess spicy, sour, oily, junk foods (aggravates Pitta-Kapha).

    • Daytime sleeping and night awakening (disturbs Netra bala).

    • Excessive exposure to dust, wind, smoke, and prolonged contact lens usage.

6. Lifestyle & Preventive Care

  • Wash eyes daily with cooled Triphala kvātha water.

  • Avoid frequent eye rubbing, overuse of contact lenses, or exposure to allergens.

  • Practice Netra Yoga (palming, gentle blinking exercises).

  • Maintain hygiene of spectacles and avoid irritants like cosmetic eye products.
