Anal Fissures – Ayurvedic Management & Treatment Protocol

1. Modern Overview

Anal fissure = small tear in the lining of the anus, usually caused by passing hard stools, constipation, or chronic straining.

  • Symptoms: Sharp pain during defecation, bleeding (fresh blood), spasm of anal sphincter, constipation, burning sensation.

  • Modern management: Laxatives, stool softeners, local anesthetics, sphincter relaxants, surgery in chronic cases.


2. Ayurvedic Understanding

Anal fissures are correlated with Parikartika described in classical texts.

  • Doṣa involvement: Vāta (cutting pain), Pitta (burning, bleeding), Kapha (chronicity, induration).

  • Nidāna: Ati rukṣa āhāra, vāta-pittaprakopaka ahāra, chronic constipation, strain.

  • Samprapti: Vāta prakopa → Kathina mala (hard stool) → tearing of guda mārgā (anal canal) → rakta srāva + sphincter spasm + vedanā.


3. Chikitsā Sūtra (Principles of Treatment)

  • Vāta-Pitta Shamana (relieve pain & burning).

  • Mridu Virechana (soft evacuation, prevent recurrence).

  • Bṛṃhaṇa & Ropana (healing of fissure).

  • Local Snehana & Avagāha Sveda (reduce spasm & pain).

  • Rasāyana (prevent chronic recurrence).


4. Panchakarma Protocol

  • Avagāha Sveda: Sitz bath with Triphala Kaṣāya, Pañcavalkala Kaṣāya → relieves pain, improves circulation, promotes healing.

  • Snehana & Basti:

    • Mātrā Basti with Kṣīrabala Taila or Yashtimadhu Taila (softening stools, healing fissure).

    • Anuvāsana Basti in chronic cases with Jatyādi Taila or Balataila.

  • Mridu Virechana: With Avipattikara Chūrṇa or Trivṛt Lehya to regulate bowel.


5. Oral Medications

  • For Constipation & Smooth Evacuation

    • Triphala Chūrṇa at bedtime.

    • Gandharvahastādi Eranda Taila (mild purgative).

  • For Healing & Pain Relief

    • Guggulu Panchapala Chūrṇa.

    • Abhayāriṣṭa 10–15 ml bd.

    • Sukumāra Ghṛta internally for vāta-pitta balance.

  • Rasāyana & Ropana

    • Yashtimadhu Chūrṇa with milk.

    • Amalaki Rasāyana.

    • Haridrā Khanda.


6. Local Applications (Prayoga)

  • Taila/Malikā for local application:

    • Jatyādi Taila (classical wound-healing oil).

    • Nirgundī Taila, Yashtimadhu Taila.

  • Ointments (Anjana/Lepa):

    • Haridrā + Ghee paste.

    • Guggulu + Triphala + Honey paste.


7. Pathya-Ahāra & Lifestyle

✅ Pathya

  • High-fiber diet: whole grains, green leafy vegetables, fruits (papaya, figs).

  • Plenty of warm water.

  • Milk with ghee at bedtime to soften stools.

  • Warm sitz baths after defecation.

❌ Apathya

  • Spicy, fried, sour foods (pitta aggravating).

  • Constipating foods (red meat, cheese, refined flour).

  • Excess alcohol, smoking.

  • Prolonged sitting, ignoring urge for defecation.


8. Yoga & Prāṇāyāma

  • Āsanas: Vajrāsana (after meals), Pavanamuktāsana, Malāsana.

  • Prāṇāyāma: Anuloma-Viloma, Śītalī (cooling).

  • Relaxation: Reduces spasm & stress-related constipation.


Sample Practical Protocol (4 weeks)

Week 1–2:

  • Avagāha Sveda with Triphala Kaṣāya daily.

  • Triphala Chūrṇa at night + Abhayāriṣṭa.

  • Local application of Jatyādi Taila.

Week 3–4:

  • Sukumāra Ghṛta (internal).

  • Mātrā Basti with Kṣīrabala Taila (every 2nd day).

  • Rasāyana: Yashtimadhu + Amalaki.

  • Continue Pathya diet & Yoga.
