Anal Fissures – Ayurvedic Management & Treatment Protocol
1. Modern Overview
Anal fissure = small tear in the lining of the anus, usually caused by passing hard stools, constipation, or chronic straining.

Symptoms: Sharp pain during defecation, bleeding (fresh blood), spasm of anal sphincter, constipation, burning sensation.

Modern management: Laxatives, stool softeners, local anesthetics, sphincter relaxants, surgery in chronic cases.
2. Ayurvedic Understanding
Anal fissures are correlated with Parikartika described in classical texts.

Doṣa involvement: Vāta (cutting pain), Pitta (burning, bleeding), Kapha (chronicity, induration).

Nidāna: Ati rukṣa āhāra, vāta-pittaprakopaka ahāra, chronic constipation, strain.

Samprapti: Vāta prakopa → Kathina mala (hard stool) → tearing of guda mārgā (anal canal) → rakta srāva + sphincter spasm + vedanā.
3. Chikitsā Sūtra (Principles of Treatment)

Vāta-Pitta Shamana (relieve pain & burning).

Mridu Virechana (soft evacuation, prevent recurrence).

Bṛṃhaṇa & Ropana (healing of fissure).

Local Snehana & Avagāha Sveda (reduce spasm & pain).

4. Panchakarma Protocol

Avagāha Sveda: Sitz bath with Triphala Kaṣāya, Pañcavalkala Kaṣāya → relieves pain, improves circulation, promotes healing.

Mātrā Basti with Kṣīrabala Taila or Yashtimadhu Taila (softening stools, healing fissure).

Anuvāsana Basti in chronic cases with Jatyādi Taila or Balataila.

7. Pathya-Ahāra & Lifestyle

High-fiber diet: whole grains, green leafy vegetables, fruits (papaya, figs).

Plenty of warm water.
-
Milk with ghee at bedtime to soften stools.
-
Warm sitz baths after defecation.
❌ Apathya
-
Spicy, fried, sour foods (pitta aggravating).
-
Constipating foods (red meat, cheese, refined flour).
-
Excess alcohol, smoking.
-
Prolonged sitting, ignoring urge for defecation.
8. Yoga & Prāṇāyāma
-
Āsanas: Vajrāsana (after meals), Pavanamuktāsana, Malāsana.
-
Prāṇāyāma: Anuloma-Viloma, Śītalī (cooling).
-
Relaxation: Reduces spasm & stress-related constipation.

Week 1–2:

Avagāha Sveda with Triphala Kaṣāya daily.

Triphala Chūrṇa at night + Abhayāriṣṭa.
