Core Principles of Basti Therapy
Basti (enema therapy) is considered the supreme treatment for diseases stemming from an imbalance of Vata. Because Vata is responsible for the movement and elimination of all bodily waste (including fecal matter, Kapha, and Pitta), managing it through the rectum is the most efficient way to maintain physiological balance. The therapy is categorized into three primary types:
• Niruha (Asthapana) Basti: A decoction-based enema involving salt, honey, and herbal elements.
• Anuvasana (Snehabasti) Basti: A fat-based enema using oils or ghee, typically utilized for lubrication.
• Uttarabasti: A specialized enema administered through the urethral or vaginal routes.
Guidelines for Administration
The procedure requires strict adherence to patient readiness, nozzle specifications, and dosage.
• Patient Suitability: Niruha Basti is ideal for conditions like abdominal tumors, chronic fever, gout, and various Vata-related obstructions. It is contraindicated for those with severe emaciation, recent diarrhea, acute respiratory distress, or pregnancy (in the seventh month).
• Technical Precision: The nozzle should be crafted from durable materials like metal, wood, or bamboo, shaped like a cow's tail to ensure smooth insertion. Measurements for the nozzle length and diameter vary by the patient's age and physical constitution.
• Dosage: Quantities are highly individualized. For example, a child of one year requires one Pala (48g), while an adult typically requires 24 Pala (1152g) of decoction. Anuvasana doses are generally one-fourth the quantity of the decoction enema.
Procedural Regimens
Different treatment cycles are defined based on the patient's condition:
1. Karma Basti: A comprehensive course of 30 enemas, alternating between decoction and oil enemas.
2. Kala Basti: A 15-enema regimen.
3. Yoga Basti: An 8-enema regimen.
A vital takeaway is that neither fat nor decoction enemas should be used in total isolation. When used alone, they may cause nausea or unintended Vata aggravation; when combined, they provide the necessary balance of purification and lubrication.
Specialized Applications and Scope
• Uttarabasti: Reserved for reproductive and urinary tract disorders. For women, this is preferably administered during the menstrual cycle when the uterine path is naturally open.
• Matra Basti: A low-dose, daily fat enema suitable for children, the elderly, and those accustomed to heavy physical labor or exercise. It is highly comfortable and does not require the strict dietary regimens associated with larger procedures.

Because it addresses the root of systemic movement and waste regulation, it is frequently described as Ardha Chikitsa—representing half of all possible medical treatment—while some scholars assert it is so foundational that it constitutes the entire scope of healing.
