Elevated Intraocular Pressure and Vātānubandha Abhishyanda: Ayurvedic View of Glaucoma

Elevated Intraocular Pressure and Vātānubandha Abhishyanda: Ayurvedic View of Glaucoma

1. Modern Explanation:

Glaucoma is a group of eye diseases marked by increased intraocular pressure (IOP), which gradually damages the optic nerve, leading to permanent vision loss if untreated.

There are types like:

  • Open-angle glaucoma (chronic, slow)

  • Angle-closure glaucoma (acute, painful)

  • Normal-tension glaucoma

  • Congenital glaucoma

2. Signs and Symptoms:

  • Gradual peripheral vision loss (tunnel vision)

  • Eye pain or pressure (in acute cases)

  • Halos around lights

  • Redness of the eye

  • Headache, nausea (if IOP spikes suddenly)

3. Ayurvedic Correlation:

🟢 Best correlated with:

  • Adhimantha (अधिमन्थ) – described as a painful eye condition due to doshic accumulation leading to vision impairment

  • Also associated with Vātānubandha Abhishyanda (Vāta dominant inflammatory eye disorder with discharge and tension)

If untreated, Adhimantha leads to Linganāśa (blindness) in 10 days per classical references.

4. Doṣa–Duṣya–Samprāpti:

  • Doṣa: Predominantly Vāta + Pitta/Kapha (depending on type)

  • Duṣya: Rakta, Majjā, Rasa

  • Srotas: Raktavaha and Majjavaha srotas

  • Samprāpti: Vāta gets vitiated → obstructs ocular channels → raises pressure → damages optic structures

5. Śloka Reference:

Suśruta Uttara Tantra 11/1–2:
"त्रिदोषैर्भवति व्याप्तं नेत्रं यस्मिन् स तीव्ररुक्।
अधिमन्थस्तु स विज्ञेयः..."

🔹 When the eye is afflicted with all three doṣas, and intense pain prevails, it is known as Adhimantha.

Suśruta Uttara 11/3:
"स चैव दशमेऽहनि लिङ्गनाशं प्रयच्छति॥”

🔹 If untreated, it can lead to blindness within 10 days.

6. Ayurvedic Management:

🔹 Local Kriyākalpa:

  • Āśchyotana: Triphala decoction with Yashtimadhu

  • Anjana: Rasāyana Anjana with Chandrodaya Varti

  • Netraseka: Chandana Bala Lodhra Kashaya

🔹 Internal Medications:

  • Saptamrita Lauha – with ghee and honey

  • Punarnavadi Kashaya – to reduce eye pressure and oedema

  • Pathyadi Kashaya – for headache and eye strain

  • Punarnava Mandura / Chandraprabha Vati – for fluid regulation

🔹 Panchakarma:

  • Nasya: Anu Taila / Jeevantyadi Taila (nourishes optic nerve)

  • Virechana: Mild purgation for Pitta-Vāta control

  • Basti:

    • Tikta Ksheera Basti – for chronic eye conditions

    • Dashamoola Niruha Basti – reduces systemic Vāta

🔹 Rasayana & Support:

  • Triphala Ghrita, Chyawanprash

  • Ashwagandha, Yashtimadhu, Brahmi Ghrita

🔹 Pathya:

  • Warm ghee, green gram soup, barley, amalaki

  • Avoid: Day sleep, emotional stress, alcohol, spicy & dry food

📝 Summary Insight:

In Vātānubandha Glaucoma, managing ocular tension and nourishing Majjā is vital. Timely Ayurvedic intervention can prevent progression and preserve optic nerve health.
